A Living Will
A living will also called an Advance Health Directive, or an Advance Health Care Directive, is not a part of your Last Will and Testament. It is a separate document that legally records your wishes concerning treatment when those wishes can
no longer be personally communicated by you.
A living will lets your family members and your doctors know what type of care you do or do not want to receive should you become Terminally Ill, Permanently Unconscious or you are determined to be in a Persistent Vegetative State.

The living will becomes effective only when you cannot express your wishes yourself. If your state recognizes a power of attorney for health care, you may also execute a "Durable Power of Attorney for Health Care" to authorize someone to act in accordance with your present intentions. They can make medical decisions for you when you are unable to do so yourself. It is recommended that you have both documents.
Discuss your wishes as reflected in your living will with family members, and be sure they have a signed copy. Also, be sure to have your doctor place a copy of your living will and "Durable Power of Attorney for Health Care" in your medical record.
